Privacy Policy
All data concerning the treatment are registered in the patient administration system and in the administration program. These regulations are drawn up in accordance with the “Personal Data Protection Act”.
The following applies to accessing patient data:
- The practitioner maintains the data;
- The practitioner has access to all data, assistants only have access to medical data for current treatments;
- A colleague only has access to the patient file for a reason, such as replacement, peer review, etc.;
- In the event of a transfer to another healthcare provider, reports will only be sent after the patient's consent. Patient approval will be recorded in the patient administration system;
- The name and address details (incl. BSN number) for the practitioner and assistants are available via the computer. When leaving the computer, the name and address details are made inaccessible by the practitioner (the patient is put on inactive status);
- If consultation with the referrer takes place, this is always done in consultation with the patient. Patient approval is recorded in the patient administration system;
- Patients are not allowed to operate the computers so that they do not have access to other patient data;
- The patient, upon written request, has the right to inspect his/her patient file at any time;
- The patient has the right to a copy of his/her patient file.
- Patient data will be retained for at least 15 years after the last treatment date.
- Destruction of patient data is done by shredding or by a certified institution.

Your privacy is respected by us. De Schans Tandartsen strives to safeguard your privacy as best as possible and will treat the information you provide us with confidentially. When processing personal data, we observe the applicable laws and regulations in the field of privacy. In this privacy and cookie statement, we inform you about the way in which we handle your data.
Categories of personal data
By using the website and the services available on it, you leave certain data with us. This also happens in our practice in the context of the execution of the treatment agreement. This may be personal data. We only store and use the personal data that is provided directly by you or of which it is clear when provided that it is provided to us for processing.

Basis for data processing

We process your personal data because this is necessary for the execution of the agreement between you and us, as laid down in article 6 paragraph 1 sub b of the General Data Protection Regulation (GDPR). In addition, we may process your personal data for another legitimate interest, such as informing our patients about current events or changes in our services. This basis is laid down in article 6 paragraph 1 sub f of the GDPR.
Purposes of data processing
The personal data collected by us are used for the following purposes:
- Creating your account to register with the practice;
- Logging into your account;
- Maintaining your medical records;
- Scheduling an appointment;
- Performing a treatment;
- Keeping track of preferences you have indicated;
- Sending a newsletter (sporadically);
- Improving the quality of our services, for example by conducting surveys;
- Improving our online reputation;
- Performing other services requested by you.
Providing your personal data to third parties
In principle, we only provide your personal data to third parties if you have given permission for this. Data provision without your permission takes place if this is necessary to be able to execute the agreement between you and us or a legal obligation prescribes this.
Retention periods
We will not retain your data for longer than is necessary for the purposes described in this privacy and cookie statement, unless this is required by law. For the retention of your medical data, we use the statutory retention period of 15 years from the Medical Treatment Agreement Act. In addition to the WGBO, we also take into account retention periods resulting from other legislation if necessary.
