Referral to implantology
As a dentist, you can refer your patients for treatment by the implantologist, Babette Korteweg-Vissers. Referred patients can
do not register with us as a regular patient, either during or after treatment.
So you will always see your patient again! Send the referral preferably via ZorgMail to our email address: deschanstandartsen@zorgmail.nl.
We would like to receive the following information:
- Your patient's name, date of birth and contact details
- Recent treatment information and/or x-rays (if available)
If it is an urgent referral (for example acute pain or trauma of a frontal element), please state this explicitly. These patients will be seen with priority.
Treatment trajectory
After receiving the referral, we will contact your patient within a few days to schedule an appointment for an initial consultation. Your patient will receive confirmation of this by post or email.
After the first consultation, a written treatment plan is always drawn up. This treatment plan is sent to both you (via ZorgMail) and your patient (via ZorgMail or by post). In addition, the patient receives an informed consent and a cost estimate. If you would like to discuss the treatment plan, please feel free to contact us.
After the treatment you will receive a final report and any X-ray images via ZorgMail.
Below you will find the referral form for dentists or dental hygienists who wish to refer their patient to De Schans Tandartsen. We will only perform the treatment(s) for which you have referred the patient. For other treatments, the patient will remain under your care at all times.
